The forms listed below are those most commonly needed for completion of the doctoral degree. The sequence in which the forms are to be completed is explained under Sequence of Events, Deadlines and Forms.
- Doctoral Student Progression Report required submission twice a year to the Graduate Coordinator
- Request for Progression Extension to be used in case standard progression deadlines (except progression report) cannot be completed at the stated deadlines
- The Graduate School now uses online Graduate Academic Petition for the following: Course Overload, Course Revalidation, Course Substitution, Late Add, Late Withdrawl, Leave of Absence, Transfer of Credit, and other course related issues.
- Appointment of Doctoral Dissertation Committee
- Qualifying Exam Topic Approval Sheet includes the determined 5 areas of study for the Qualifying Exam.
- two areas associated with the required Ph.D. core curriculum (see list of 50 Core Area Choices)
- one area of the student’s specific research interest and thesis research
- one area selected by the student’s Major Advisor or Dissertation committee typically related to elective coursework and thesis research concentration of the student
- one area selected by the student typically related to elective coursework and thesis research concentration of the student.
- Report of Comprehensive/Qualifying Exam…and Study Report for Doctoral and/or Master’s students
- Student Learning Outcomes Assessment Form — Doctoral
- Dissertation Proposal Title Page
- Dissertation Proposal Defense Report
- Final Defense Report Check the box marked Dissertation.

I am preparing for my Qualifying Exam. What forms do I need?
I passed my Written Qualifying Exam. What forms do I need?
None. The written component is the FIRST part of the Qualifying Exam. No form is required until the studnet has passed both the written AND oral components.
I am completing my Oral Qualifying Exam. I am not ready to defend my Dissertation Proposal at this time. What forms do I need?
- SLO forms a separate form is to be completed by each and every member of the Dissertation Committee and returned to the Graduate Program Director
- Exam Report Indicate both major and PhD degree. Check Box marked Qualifying Exam.
Previously I completed my Oral Qualifying Exam. Now I am prepared to defend my Dissertation Proposal. What forms do I need?

- Proposal Defense Report Indicate both major and PhD degree.
